Wheezing Breath

Wheezing is a high-pitched whistling sound produced during breathing, typically when exhaling, due to narrowed or obstructed airways. It is both a symptom and a physical finding that can indicate various underlying respiratory conditions.


Causes

Common causes of wheezing include:

  1. Asthma (most common cause in younger adults)
  2. Chronic obstructive pulmonary disease (COPD, more common in adults over 50)
  3. Respiratory infections (e.g., bronchitis, pneumonia)
  4. Allergic reactions
  5. Bronchiolitis (in children under 2 years)

Less common causes:

  • Foreign object aspiration
  • Heart failure (cardiac asthma)
  • Gastroesophageal reflux disease (GERD)
  • Vocal cord dysfunction
  • Tumors in the airway

Pathophysiology

Wheezing occurs when:

  • Airways become narrowed or compressed
  • Airflow through the narrowed segment becomes turbulent
  • Airway walls vibrate, producing the characteristic sound

Wheezing is more common during expiration due to increased intrathoracic pressure and airway narrowing as lung volume decreases.


Clinical Presentation

Symptoms associated with wheezing may include:

  • Shortness of breath
  • Chest tightness
  • Coughing
  • Labored breathing, particularly during exhalation

Diagnosis

Diagnostic approach includes:

  • Detailed medical history and physical examination
  • Chest auscultation
  • Pulmonary function tests
  • Chest X-ray
  • In some cases, CT scan or bronchoscopy

Treatment

Treatment depends on the underlying cause and may include:

  1. Bronchodilators (e.g., albuterol) to relax airway muscles
  2. Inhaled corticosteroids to reduce airway inflammation
  3. Oxygen therapy in severe cases
  4. Antibiotics for bacterial infections
  5. Treatment of underlying conditions (e.g., GERD, heart failure)

For symptomatic relief:

  • Steam inhalation
  • Staying hydrated
  • Using a humidifier
  • Avoiding triggers (e.g., allergens, irritants)
