Fall Attack
Fall attacks, also known as drop attacks, are sudden falls without loss of consciousness. They can have various causes, both epileptic and non-epileptic. Here’s an overview of the etiology, pathogenesis, and differential diagnosis of fall attacks:
Non-Epileptic Causes
- Transient ischemic attack (TIA)
- Stroke
- Syncope
- Vertebrobasilar insufficiency
- Vestibular disorders
- Parkinson’s syndrome
- Orthostatic hypotension
- Carotid sinus syndrome
- Cardiac arrhythmias
- Myocardial infarction
- Musculoskeletal disorders
- Alcohol or medication effects
- Gait disorders due to neurological conditions:
- Polyneuropathy
- Myopathy
- Cerebellar disorders
- Communicating hydrocephalus
- Subcortical arteriosclerotic encephalopathy (Binswanger’s disease)
- Visual impairments
- Spinal or cauda equina claudication
- Cataplexy (sudden muscle tone loss with emotional excitement, cardinal symptom of narcolepsy)
- Psychogenic attacks:
- Hyperventilation tetany
- Panic attacks
- Hysterical attacks
- Orthostatic tremor (a form of essential tremor)
Epileptic Causes
- Complex focal seizures
- Simple focal seizures
- Todd’s paralysis (post-ictal)
- Atonic seizures: Brief loss of muscle tone, can occur with or without loss of consciousness
Pathogenesis
The pathogenesis varies depending on the underlying cause. For example:
- In vertebrobasilar insufficiency, reduced blood flow to the brainstem can cause sudden loss of postural tone
- In vestibular disorders, disruption of balance mechanisms can lead to falls
- In atonic seizures, there is a sudden loss of muscle tone due to abnormal electrical activity in the brain
Diagnosis
Diagnosis involves a thorough history, physical examination, and often additional tests such as:
- Electroencephalography (EEG)
- Neuroimaging (CT or MRI)
- Cardiovascular tests (ECG, Holter monitoring)
- Vestibular function tests
The specific cause of fall attacks can be challenging to determine, especially in elderly patients where multiple factors may contribute. A multidisciplinary approach is often necessary for accurate diagnosis and appropriate management.
